Who Needs To Be Prequalified?
National Maintenance & Construction (NM&C Inc.) requires that all vendors/subcontractors be prequalified to be eligible to bid and contract work with our company before working with NM&C Inc.
General Required Documentation
- Licensing: Meet all state and local municipality licensing requirements.
- Insurance: Meet and or exceed NM&C Inc. insurance requirements.
- Dedicated To Quality: Do the job right – the first time.
- Be Reliable: Show-up and complete your work on-time.
- Work Clean: Keep your work area clean at all times.
- Be Professional: Treat Clients with care and respect.
- Safety & Environmental Management Program: Agree to comply with our Safety & Environmental Management Program.
- Terms & Conditions: Agree to the NM&C Vendor Master Terms and Conditions.
Submit your W-9 with full legal name, signed and dated within the past year. If you need to fill out a new W-9, you may fill out the form at the bottom of this page.
Vendor will provide a Certificate of Insurance evidencing that vendor has procured and is maintaining the following insurance coverages during the term of the applicable Order and or Agreement:
(a) Comprehensive General Liability Insurance covering all operations by or on behalf of vendor providing insurance for bodily injury, property damage and personal injury liability of at least $1,000,000.00 per occurrence and $1,000,000.00 in the aggregate.
(b) Automobile Liability Insurance for owned, leased and non-owned automobiles with a combined single limit for bodily injury and property damage of $1,000,000.00 per occurrence.
(c) Workers’ Compensation and Employers Liability Insurance, as required by any applicable law, rule, regulation, statute or order.

The Certificate of Insurance shall provide that no policy providing any of the foregoing coverages may be cancelled or reduced without thirty (30) days prior written notice to NM&C Inc.
PLEASE NOTE: All certificates of insurance (COI) provided to NM&C Inc. by vendor (with exception to Workers’ Compensation) MUST name National Maintenance & Construction Inc. (NM&C Inc.), and its members, managers, officers, directors, employees and agents, successors and assigns as Certificate Holder and additional insured as follows: (download sample COI)

If your company has ten (10) or less employees, provide the number of employees and the total hours worked each year. Regardless of your company size, the OSHA 300A form is required if recordable incidents have occurred in the past three years.
